Understanding ACJ pain – Causes, Symptoms, and Treatment:

Isolated pain at the top of the shoulder can often be traced to the acromioclavicular joint (ACJ), a small joint where the collarbone (clavicle) meets the shoulder blade (scapula).

What causes ACJ pain? 

  • Trauma (either direct or indirectly to the shoulder)
  • Overload (Repetitive overhead activities, throwing sports, pressing gym exercises or manual work)
  • Age-related changes (osteoarthritis).

Common Symptoms: 

  • Tenderness at the top of the shoulder
  • Pain when reaching across the body
  • Pressing movements or end of range overhead activities
  • Clicking and grinding directly over the joint.

Treatment: 

  • Activity modification
  • Physiotherapy to maintain strength and function of the shoulder in non-provocative positions
  • Pain relief, including medication, heat or ice.
  • In persistent cases, corticosteroid injections may help reduce pain and inflammation.
  • Surgery is rarely required and is generally reserved for significant joint injury, severe arthritis, or symptoms that fail to improve with conservative treatment.
